There are 3 sets of surveys to be completed. These are developmental and reflex surveys to aid us during our child’s assessment time, and they are mostly multiple-choice questions.
You are not required, but encouraged to complete all 3 at the same time. The whole process will take only about 10-15 minutes.
You will receive an email at the end of each survey. In the email, you will find a link to continue to the next survey. You may also continue your surveys from the appointment booking page.
